Waiver

Please review this waiver and its conditions to attend and participate in this program. Please indicate your determination on each provision in the space provided below:

My child has permission to attend the Quarterback University, LLC. Enclosed is payment for the required amount to start the program. Cancellation/Refund Policy: No refunds will be given after the student has registered for the program.

In the event of any emergency in which my child requires medical care, I authorize the staff of Quarterback University, LLC to act for me and to obtain for him whatever medical treatment the staff deems necessary and appropriate. I specifically waive and release Quarterback University, LLC and any and all facility’s, its owners, staff, and sponsors, from any and all liability for injuries incurred while participating in this activity.

The aforementioned sponsors include:

1. Jefferson Indoor Sports Complex.
2. ALL Louisiana Parks and Recreational Clubs, organizations and park facilities.

Furthermore, I have no knowledge of any physical impairment that would affect or be affected by my child’s participation in the Quarterback University, LLC Program.

I authorize Quarterback University, LLC to use any photographs or articles about my child for publicity purposes. I understand that after payment there are no refunds.

No Player will be accepted without parent/guardian approval.

Note: For all Private Sessions, a QB is to bring a personal receiver. If you cannot provide a receiver, Quarterback University will try to provide one who is to be paid a gratuity of at least $15.00 after each Private Session is completed. Private Sessions can contain multiple clients, based on availability, to ensure all clients receive the proper attention.

After reviewing, you may proceed to the application page and check the box saying you accept all of the above written terms and conditions of this waiver. That box will be accepted along with the name in the form will be considered your signature of this document.
